Case Management Services
Empower Together Program - Case Management & Counseling Services
Community members who are engaged in counseling services at NYFS also have access to working with our Case Manager who has learned about many different types of resources within the community and the ever-changing challenges to locate them.
The Case Manager can assist individuals with a variety of needs including:
- Insurance Issues (health or disability)
- Transportation
- Employment
- Legal Assistance
- Shelters
- Housing
- Finances
Our Case Manager is dedicated to getting the support, service and/or information needed for the individual served. With the altering network of services available to the community, not every resource may be accessible or known. We work diligently to find the appropriate service(s) to ensure that an individual’s needs are met.
